Anal Leakage (7mth old kitten)

There is a long history for my cat, and it's a big read but I'll try to condense it. Mainly, right now, his biggest symptom is anal leakage. I noticed yesterday (Wednesday) afternoon that my kitten had the end piece of poop (the round bit that has a bit of a stringy bit attached) stuck in his anus. The skin around it was bloody, swollen, and his cat bed (covered in a pee pad for dogs, since my kitten's been sick lately) was covered in bright red blood.

I thought it was diarrhoea at first since it was the colour, so I gently used a hypoallergenic wet wipe to wipe it off. When I realised it was solid I left it alone and called my vet. The blood was alarming so while I packed things up (his huge folder from frequent ER vet visits - yes, he's been that sick - and medications), the vet told me if he passes the stool, baggie it and bring it. He passed it while I used Feliway in his carrier and then we made the trip to the vet.

Now, 24 hours before I noticed this half-in, half-out poop, he'd had his anal glands expressed. He previously had a vet visit as a result of sudden diarrhoea, lethargy, vomiting, rapid weight loss and other symptoms that led the ER vet to tell me I needed three tests done ASAP: FIV, FELV and toxoplasmosis. The latter test was done first, with a CBC as my kitten (Roman) tends to become hypoglycaemic and anaemic. CBC came back fine, but he shows a previous toxoplasmosis infection and a non-shedding one currently. He's being treated for this.

The FIV/FELV test was run Tuesday and I'll have it's results this week, but his symptoms (seizures, etc) are leaning towards FIV. He was born early April, 2013, so he is quite young. I did get him from the SPCA, and my friend works there and knows the woman who adopts off most days, so we've all been in close contact regarding this, considering Roman was very ill with diarrhoea and vomiting both a few days after I adopted him and a few days before (during which time he was spayed/neutered and given his first shots/rabies shots). I should note he was very sick and horribly underweight during this.

The reason he started going to this current vet was because the current vet I was at had no idea what to do for his behavioural issues, which seemed a mix of misplaced aggression and separation anxiety, all of which happened after the first bout of illness left him in hospital for five days on IV (I came to visit twice a day for an hour at a time). Now, when we started to look at behaviour, be exhibited other symptoms of illness which has led to where we are now: something is clearly wrong.

My main concern is that he is leaking semi-soft/solid diarrhoea coloured poop from his anus. It's small, maybe the length of a fingernail (like a rabbit's pellet poop). He is  going in the litter box and having dark, normal, solid poops but it seems there is some he either cannot get out in the box or is leaking out. I'm going to be calling the vet in the AM, but considering his bills have run up 4000$ CDN in less than two months, if it is not dire, I cannot bring him in seeing as how the vet has him on everything possible - anti-diarrhoea, etc. I'll list his meds at the bottom.

I was warned that his symptoms are very close to end-stage 1 FIV, and that it is possible this will be the test that comes back positive. Until I get those results, I'm curious if anyone else has had this issue. When he does pass the leftover stool, his anus is bright pink around where he passes, and sometimes there is blood (latter of which I was told is the result of a tear inside that the vet found). 

Sorry this is TMI and all over the place, but I don't know what else to do and his declining health had my vet gently tell me that I should look into 'aftercare' because the way his health and failing immune system is going, my baby boy may not make it into the New Year.

Weight: 2.6kg (yes, he's underweight despite frequent large feedings)

Breed: Russian Blue - Siamese mix (domestic shorthair)

Roman's Meds (current, 12/12/2013)

Lygine Powder  - upper respiratory (herpes virus)

Metronidazole - anti inflammatory for upper GI

Antirobe (clindamycin)  - antibiotic for toxoplasmosis

Fortiflora  - probiotic to regulate gut bacteria

Clomipramine - anti-anxiety

ETA; Roman is an indoor cat, has never been outdoors. Each blood panel had him tested negative for worms, parasites and the like. The main and only concerns in blood work now are FELV or FIV.

@LDGPoor little guy. He does seem to have some serious health issues right now, but he is very lucky to have you taking care of him. FIV is not an automatic death sentence like your vet is seeming to suggest. Member LDG has a ton of information and the latest research on caring for FIV+ cats. Hopefully she can chime in here.

This does not sound like a typical diarrhea case to me. Does he seem to strain while pooping at all? The leaking almost seems like constipation, but if he's pooping normally, then I really don't know. What is diet like?

Most cats with FIV aren't symptomatic this early in life (FeLV is another story).  Most cats are tested with a 10 minute snap test so I'm curious why yours is taking so long.

if he hasn't been outdoors and hasn't had contact with other cats, the only way he could get FIV/FeLV is from the mom.  Is there a test history for her at your SPCA?
