My cat is suddenly afraid of everything....

Looking for some advice and ideas on what could be causing this...I have a vet appointment on Monday and would like to have a clue on what questions to ask.

Here is the story....We adopted our cat Berry in May 2012 so now he is about 16months old. (he is an indoor cat only)  Two days before Thanksgiving last year he ended up in the Vet ER because he had a heart attack.  The vet explained he has an enlarged heart and is on Medication for the rest of his life.  He is in Furosemide and Enalapril.  I took he to the vet again 10 days ago for his rabies and distemper shot.( I run a daycare and need to make sure they stay up to date with all the shots and like to be sure incase he ever gets out we are covered)  When we got him home he was really cautious with everything he did.  I figured this is pretty normal for him. 
Well he hasn't gotten better.  He is afraid of everything he looks up, down and all around with everystep he taking.  He is constantly sniffing everywhere.  He looks anxious all the time and seems to keep getting worse.  Last night he peed and pooped all over my couch and he has never had an accident in his life. He has not played in days.  He is still social and will curl up with us but the slightest movement or noise will send him running.  He usually comes running for his medicine, instead we had to seek him out.

Berry is normally a very laid back cat who is not afraid of anything and plays with everything.  At night I have woken up many times thinking someone was breaking in the house and it was just the cat playing.  he lets everyone pick him up, he lies down right in the middle of the kids games,  He has lied on the kids blankets and they have pulled him around the house and he doesn't even bother to open his eyes. 

Any advice on what could be causing this and what questions to ask when he gets to the vet would be great!  Thanks

Well the behaviour could be many things he may be having an adverse reaction to the vaccines, it could be his medication and the dose needs to be adjusted or it could be something else entirely. A vet trip is in order to determine what is going on

One piece of advice I can give you is that next time I would get his vaccinations spread out and not given at the same time. In my opinion vaccines can be pretty hard on some cats and multiple vaccines at one time causes more stress on a cat's health. Thanks for the info.  The other thing that may play a factor is we have an unneautered male cat hanging around the yard now and again.  I wonder if that cat is spraying outside because the couch he went on is right near a window we keep open and by the bush I saw this other cat near.  Oh so many things that can factor into this.

Oh yes that for sure could be the reason. If this is a behaviour issue and not related to health I would advise getting a feliway diffuser and you can also look into using spirit essences (http://store.jacksongalaxy.com/category-s/1849.htm)

Update: On Berry,   I am still completely confused about my cat.  He has made his "headquarters" on my bed.  He lies there all day.  I have had to move his litter box from the upstairs bedroom to my room (My room is the Finished basement of the house).  I have had to move his water and food there too.  I have it on the bedside table so he doesn't have to go far.  His anxiety has seemed to calm down a bit as long as he is on the bed.  We picked him up and put him on the floor and you could see his anxiety immediately appear. so we picked him up and put him and he is calm.  Every time I run downstairs to check on him he is sitting up proudly on the corner of the bed, I go over to him and pat him and he instantly purrs.  When I leave he goes back to the other corner and lies back down.

He is eating some (I don't think as much as usual but he is eating).  I brought him tuna juice and looked somewhat excited and drank it all.  he has been drinking and taking his meds.  I almost feel as if I am enabling his behavior by waiting on him like this but, it make me nervous when he gets anxious, I don't want him to go into congestive heart failure again like he did at Thanksgiving.

I have also sprinkled a small animal repellant around the perimeter of my yard but, away from my windows and doors, to try to keep the stray away . But he wont even think about coming upstairs.

This vet appointment on monday can't come quick enough.
