Depends upon the type of worm(s).  Some worms are notoriously difficult to get rid of (girardia), and it may not be worms you're dealing with, but bacteria, like e-coli.  It may take several courses of de-wormers to take care of the problem.  Did the vet tell you what kind of worms she had?

Meanwhile, how is her appetite?  You can mix some kitten replacement milk with wet food to get the calories into her.

Good luck--and thank you for taking care of this little one.

JadePrincess, You do weigh her every day? Best in grammes. It is the easiest way to notice changes.

What food do you give her?

What preparate for deworming did you used? Usually they begin to work quite quickly, two day perhaps, and you notice dying worms coming out. The poo gets often runny too.

What worms do you suspect? Kittens has almost always round worms, sometimes also tape worms.

You must often do it again a couple of weeks later.

And not seldom they do have other parasites, like Giardia...  But this is perhaps another story.

Please, continue to report and question!

Good luck!

If she ates wet it should be OK, even if it is not the small kitten variation.  I mean, its easier to make a decent wet food than decent dry food.

The danger is if it is some low fat variant; kittens shall have much fat.  Full fat variation.

If it is a low fat variant, you can try and melt in some butter.

Or mix it with this "kitten under 4 months" you do have.

Milk (kmr or goat milk) as extra is also good,

Good luck!

What dewormer did you use? It's very important you don't buy over the counters at a pet store as usually they do nothing. Its also important that you deworm again in two weeks depending on what dewormer you are using. Her tummy is very distended is she having normal bowel movements and is she having any diarrhea or loose stools? Any vomiting? Is she active?

As someone else mentioned, pretty much all kittens have round worms so at the very least need strongid or drontal once and then again in two weeks. Over the counters don't work but all kittens need to be dewormed.
