I took my kitty to the vet last Monday to get neutured. They vaccinated for rabies and gave him a 3-day pain killer shot. As of yesterday and today, he keeps getting sick. Bile and loose stool, simultaneously. Should I be worried?

Some tummy upset is normal after surgery or vaccines (and he got both!). But it's been 4 days. . .you should probably call the vet to ask about it. Is he eating and drinking well? Can he keep anything down?

YES!  Please call the vet or get to the emergency vet ASAP.  Your kitty could be having a reaction from the medication and/or the rabies shot.

THIS.  What you are describing might  be normal for 24 hours after shots, but is no longer normal.  Please please get him to a vet.

I don't recall the name of the shot. They just claimed it would last for a full 3 days. They told me that he could have these issues related to the pain meds  but, it didn't start until it should've been wearing off. Not at all a good sign.

UPDATE:

My little man had to get IV fluids and antibiotics for infection. He is all tuckered out here while getting his fluids. Poor baby.

Poor guy!  What did they say was wrong with him?  Did he have to stay at the vet?

Infection from neuter. No need to stay. Gave him IV, nausea meds and antibiotics. He should be fine!

Those are the IV fluids.  His body will gradually absorb them and this will keep him hydrated.  Is he eating yet?  His appetite might be off a bit due to the fluid intake, but he should start eating now.  To encourage him to eat, you might get some canned food and warm it in the microwave for a few seconds to enhance the smell.  You could also try adding a touch of tuna juice.  Also, plain meat baby food is good for times like these.  Be sure it's all meat, no dinners, no onions, and no garlic.

Did he get fluids IV or subcutaneously? SubQ leaves a mound of fluids, IV shouldn't.

One of my boys got infected after his neutering 1.5 years ago. Antibiotics cleared him right up. And yes, make sure he eats. He needs nutrition to heal.

He's eating already. However, he hates his medicine... the vet didn't give enough medication in the event he spits it all out! I give it to him and then he clamps his jaws shut and it all comes spilling out the sides.

Gah!!!
