Help and advice needed please x

Hi there, I don't know how far gone my cat is? She is rugby ball shape, enlarged pink nips, yellow milky goo from her behind and she also is enlarged from her behind. She has a big appetite drinking plenty which is gd,, she goes out then comes back in when just going outside, and obliously don't want to be on her own as she always stays in the room I am in and follows me and and always sleeps.
