UTI issues

Back in December, my male cat had a UTI.  Our vet gave him a shot of antibiotic because we told them it is hard to pill him.  The vet said that they like to give two shots 10 days apart to make sure it really works.  So we did the 2 shots and less than a month later he is peeing blood again.  We took him in last week and they couldn't get any urine but decided to give another shot.  Today he peed on the floor again, he hasn't done it since the last shot.  I'm assuming the UTI is back?  Or something else is wrong.  Thoughts?  Should I have the vet do some other testing, if so what kind?  We did put him on the Science Diet bladder control food, he eats it but also eats the Nutro Max that our other cats eat.  I'm worried that it is something else.  He used to be the "boss" but now he is running from the other two cats so I wonder if they see him as weak?

Your poor boy! The vet should know how to do a cystocentisis which is going directly into the bladder with a sterile needle to draw out some urine. I would run a urine sensitivity culture on his urine right away. If your vet hasn't suggested this, it might be time to get another vet's opinion. The urine needs to be checked for crystals or stones. The antibiotic shot was probably Covenia and I am not sure if that is good enough for a bacterial bladder infection. Not all bacteria respond to all antibiotics. That is why IF it is bacterial to find out which kind of bacteria. Your boy needs to be treated quickly as this could potentially turn serious very fast not to mention painful and it already seems to me to be at the serious point. If the other two cats are chasing him, he might need a quiet place to recover after the new vet check up. Stress in cats can bring on blood in urine too. In December the vet did take urine directly from the bladder, there were no stones or crystals.  I believe you are right about the shot being Covenia, that sounds like what they said.  This most recent time, there wasn't enough urine in the bladder to get a sample so with the recent diagnosis, they just gave the shot.  We go to a vet practice with numerous doctors so we have seen a different one each time.  So I feel we got some differing opinions.  I was just concerned last time that since there was not enough urine to test, and that it doesn't seem to be going away there may be need for other testing.  I will ask about the bacteria.

As for the chasing it is not frequent, it is just that for a cat that went from being in charge always to now the 2 others are not giving in to his authority and sometimes chasing him, I thought that might be a problem.  Thanks.

Sounds like your cat is in pain (running away) Convenia is not an effective medication for urinary tract infections and clearly it is not helping your boy.

Dry food, any dry food, is the worst thing you can feed a cat with urinary tract problems

I recommend: you change his diet immediately to wet food and find another vet. A different practice I mean. Different vets in the same practice all recommending the same treatment, that isn't working, is not really a second opinion. :)
