Pretty much 100% she is pregnant.  If she is just back she's not quite pregnant yet and can be spayed - the sperm won't have yet met the eggs so there are no embryos, yet.  If it was up to 5 weeks ago she could easily have a spay-abort.  Not nice, but then neither is the fate of many cats and kittens in rescues who can't find a home fast enough...

Any unspayed cat who's been outside is almost certainly pregnant. It does take 3-7 days for them to come out of heat afterwards, some can also come out of heat and back in while being in early pregnancy.

Please vet her asap and get her spayed, there's no need for her to have this litter.

It is not massive pure unluck so many females "happen" to go missing when in heat. Or somebody happens to let them outside.

They WANT to go outside, they do everything to sneak out.  Especielly if they smell a tom outside.

And there ARE toms outside, as they do smell a female in heat.

The only real defence is. If you are a breeder, do have double doors and keep watch.  If you arent a breeder, spay them as soon as feasible.    :)

I too would recommend a spaying here, now.

Good luck!
