Recurrent bald spots/overgrooming

Hi! I adopted a 9 year old cat in March from foster care. He was from a colony near where I live. His name is Poptart. When we adopted Poptart, the foster care was having a fungal outbreak, and he had a scratch on his cornea. We had some topical stuff for the fungal outbreak (several cats had it) and his eye was treated by the vet. 

It's been almost a year, and Poptart has been to the vet at least 5 times. The "fungal" outbreak went away, his fur seemed to nearly grow back. And then he developed a bacterial infection in about the same spot on his skin, on his back legs and towards the tail on both sides. The skin got bald, raw and bumpy, very different from the original fungus problem. That was treated by the vet and seemed better. Now, he still has those bald-ish spots, but he's taken to licking himself so much that he has had two lesions, big raw spots, on his tummy. We had his kidney function tested and he got a steroid shot-- seemed better almost immediately. Then, 6 weeks later, a new lesion! More bald spots!

We've switched to a grain-free food. Currently we are tapering oral prednisone to see if we can fix it. We have another cat who is completely healthy but somewhat territorial about the stairs. They were introduced slowly and we have 3 litterboxes. We did try a soft e-collar. He sat and tried to lick himself (but licked the ecollar) for hours and ended up hurting his tongue on the nylon. His numerous exams and blood panel all indicated that he is healthy except for his skin issue. 

Does this recurring problem sound familiar to anyone? It has been almost a year now, and these same spots keep getting better, getting worse, etc. 

What might be causing Poptart's overgrooming? Does this sound like an allergy or stress or something else?

To me it sounds like stress. Have you spoken with your vet about trying an anti-anxiety medication?

Also sounds like stress or a reoccurring behavior (he's licked for so long, he keeps licking, even though he doesn't have a reason) to me. :nod:

It almost sounds like stress to me as well. Have you tried using any Feliway plugs in the house to possibly calm him? Otherwise, you could try some of the Natural Balance L.I.D wet food to see if any of those help?

You are absolutely sure fleas are not an issue?  If he has flea allergy just one bit will set him off.  Responding to steroids does suggest an allergy of some kind.
