I took in an amazing stray cat three years ago.  I initially thought she was pregnant because she was so large, and after taking her to the vet, she was spayed and just a fat cat! I have had her on prescription diet food and she's lost a little weight.  She was great the first two years and in the last year or so, she has had some serious anal issues.  I took her to the vet right away thinking it could be something very serious.  They expressed her anal glands and said they were full, but she continued to smell.  The vet said she may not be able to clean herself so well because she is heavy (but she's smaller than she originally was and could clean herself fine then).  I have just been trying to keep her clean and take her back for regular anal gland expressions.  They have checked her feces for problems, but she's healthy.  Over the summer I took her to another vet, just to see if they had any other ideas and they couldn't figure out why it was happening either.  She had to have surgery this summer to remove a cancerous mammary gland and about a week after that, she stopped stinking for probably two months.  However, it's been back full force the last several months.

It's basically a wet area that leaves brown spots on blankets and smells horrible.  I don't know if she is passing her hair or if it's just becoming matted and coming out when I clean it.  She poops consistently (if not a lot) and it's not soft.  I am clueless, even after an anal gland expression, it's still stinky!  Love her to pieces and still let her sit on my lap, but it is a little gross :).

Any help would be greatly appreciated!

Also, I took her to the vet (after she began stinking again) to get her x-ray and make sure the cancer hadn't returned and she had a clean bill of health!

They way you are describing it, still sounds like anal glands.  If she is leaving spots of brown liquid that smells, she may not be completely getting emptied or she just fills back up REAL fast.  What kind of food is she on?  You could try something that has more fiber in it I believe.  I know they had me trying canned pumpkin on my kitty for more fiber.

Thank you for your response!  It's weird because the anal glands just continue to stink the very day I take her home from having them expressed.  And many times the vet will tell me that they actually weren't full.  I have her on the Prescription R/D which has a lot of fiber...hmmm, just at a loss!

Sounds to me like a change of diet is called for, to get some weight off her, and to help with the anal glands.  If her stools are soft then anal gland problems are commoner.

You don't say what you feed her, but suggest you go to the Nutrition forum to get more info.

I started a post this morning regarding my Bridget's stinky butt.  I noticed that, after holding her while at the vet's, I have a brown stinky spot on my jeans.  When I read your post it made me wonder if there could be something going on with your kitty besides her anal glands.  I don't know how to direct you to my post; just look for the one about Bridget's labia issues that was started this morning.  Might not even be remotely similar problems, but you never know.
