Episioplasty

Has anyone here ever had this done to their cat? I'm seriously considering it. My Ripple, 14 yrs old, has been having issues for the past 2 years. She's overweight and can't clean herself which has led to multiple infections. One vet recommends episioplasty, the other doesn't - at the same practice where I work. So for the past year I have shaved her weekly, wash her bottom daily, and still she is raw and irritated. I didn't consider the surgery much until a couple of weeks ago when a client had this done to her dog. (removing the extra skin around the anus/vulva area) After washing Ripple this morning and seeing how irritated her skin is, I'm wondering if the risk outweighs the risk considering her age. Other than being overweight and this hygiene issue, she is healthy - bloodwork, etc. Of course I'd do testing before the surgery if I decide to do it. 

Any thoughts are greatly appreciated.

I "knew" (from another forum) someone who did it for her dog, because she wasn't properly developed and kept getting infections. And if this is due to anatomy I suppose there wouldn't be much choice. But if it's mainly a matter of her weight. . .I'd definitely try some more aggressive weight-loss techniques first.
