I don't know what to do.   I have had my cat for 7 years and he has developed kidney disease and already has allergies and chronic colitis.   He is a lot of care and I am growing very tired of this.   He doesn't poop in his box and has semi-solid feces all over the basement floor, pees outside the boxes in the basement quite often, but thankfully not upstairs yet.   I have the whole basement lined with heavy plastic so it makes the clean-up easier though.   He is on a lot of drugs and has to have an aranesp shot (very expensive) every six weeks so he doesn't become anemic.  He doesn't act like he is in a lot of pain, and is able to climb the basement stairs so I would feel very guilty putting him down.   I just can't take the care anymore.   I just feel like taking him off all of the drugs and letting him die a natural death, but don't want him to suffer and I know this will.   He does live upstairs with us except for at nite when he goes into the basement.

Where are you located?  Maybe someone can assist you with caring for him.

I would not want to put him down either.  There could be many reasons for his not using the litterbox.  How often do you scoop it?  Change the litter?  How many boxes do you have for him?

What kind of diet do you feed him?  Does his kidney disease require sub q fluids?

Have you consulted with another vet?  Checked to see if there are any organizations that will help you with the cost of the meds?

This is a great support site.  I hope others here will be able to help you find some answers and offer suport.
