4 yr. old kitty - not eating well, elevated creatinine, normal BUN

We live in Thailand, and I could really use some advice.  Our indoor, spayed, 4-year old sweetheart, Sumi, suddenly stopped eating a few days ago.  Around the same time we noticed her eating was much less, she started obsessively trying to urinate every few minutes.  The vet thought she had cystitis.  This is the second time this has happened in the last 6 months.  The vet put her on an antibiotic, and gave her a shot for pain.  This worked well last time.  This time, while it almost immediately stopped the trying to urinate, and her litter habits went back to normal right away, however, she refused to eat, looking nauseated if we put wet food in front of her, and ignoring dry food entirely.

The vet then took blood.  All things came back normal, liver function, CBC, etc., her other two kidney function tests, but her creatinine was marginally high (3.1 if memory serves).  The vet gave her subcutaneous fluids, the cat felt better, ate at least some, until the next morning, when she again didn't want to eat.  We've now done sub-C fluids twice in two days - both times, she came home, ate modestly, but by morning, again was not eating.

The odd thing is this - we had another cat die of renal failure.  But this cat is very young.  On top of that, she had no signs of any CRF - no excessive thirst, urination, no ammonia smell about her mouth.  She still is bright eyed, wants to play, has a shiny coat and is grooming herself thoroughly - all things my other cat did not do once she was seriously ill.  Obviously she needs to eat.  The vet says possible CRF starting acutely, and that some cats just stop eating at a fairly low level of elevated creatinine.  But given where we are, her young age, and the lack of other symptoms, I'm really not sure what to believe or what to do.  

Other medical info - she has feline leukemia since being an infant on the streets of Ethiopia.  She has no symptoms however, and has had interferon, and her blood counts have been great.  She does not go out.  She eats a mix of Whiskas dry cat food and a local gourmet tuna wet food that the spoiled baby insists upon.  Non-tuna wet food is hard to find here, and she turns her nose up at the prescription diet stuff the vet has, and it never seemed important to insist until now.  She also has had chronic asthma since she was a few months old.  Since moving here where vet care is better, she's stopped coughing and looked incredibly healthy.  She is on a bronchodialator with weak diuretic that she takes twice a day and a very very low dose of prednisolone that she takes once a day (about 1 mg I think though I'd have to check - i think the little pink pills are 5 mg and the vet prepares 1/4 size pills - tiny things - for her to take).   She also is taking a feline topical heartworm medication once a month as it's apparently a problem for cats here even indoors given our mosquitoes.  Otherwise, she's been a happy, healthy, affectionate, playful girl, so we're puzzled and upset as suddenly it feels we could be losing her, and very quickly.

Does anyone have any experience or advice that might help us learn what to do for her?

Sorry - just got back from the vet - the correct reading for her creatinine was 2.5.  The vet does not think this is related to her feline leukemia as she has had no fever, or any other sign of illness or infection and her blood cell counts are normal.
