20 year old cat with Bile Duct Blockage, Jaundice. Cholangiohepatitis? Please Help!

Hello, I’m new here. I found this site researching information for my 20 year old cat Squeaky. I am hoping someone can provide me with some answers.

A week ago he suddenly became very ill. He was disoriented, wouldn’t eat and was very lethargic. I rushed him to the ER and his blood work revealed high levels of bilirubin and liver counts. They gave him fluids and I took him to his regular vet the next day. X-rays & Ultrasound were performed, which revealed inflammation, bile ducts blockage and an enlarged liver. He became jaundiced very quickly and is having trouble breathing. He was given Convenia (antibiotics) and Ursodiol (to unblock the bile ducts) and fluids. The next day he took a turn for the worst and another X-ray revealed a significant enlargement of the liver and fluid retention. He was given Lasix (diuretic) and the vet said he would not live much longer than 48 hours. I suspected a reaction to one of the drugs.

That was last Friday and Squeaky is still with us. We continue to give him fluids every day and he drinks water on his own, but will not eat. His breathing improved and he became perkier. Today, for the first time in a week he ate about a tablespoon of chicken baby food (a suggestion from a friend who gives that to her cat when she is ill). I was so happy that he started eating again. However, a few hours later, he went downhill again. He is having difficulty breathing and is very lethargic. I’m afraid his condition has worsened because of the food. Perhaps his bile ducts are still blocked? I’m afraid to give him the Ursodiol again because of the reaction he had the first time.

I’ve had three different vets say there is not much we can do for him due to his age. I do not want to give up on him. He is a fighter and has been through some tough times in his life, all of which he beat the odds.

Are these bouts of doing better and then doing worse normal for this disease? Does it get better even if he isn’t eating? I have so many questions. Any suggestions or thoughts on his condition would be much appreciated.

I am so sorry your baby is going through this....
This is a difficult situation because of the age, however on thing is for certain, you have, no matter what, to feed this kitty, otherwise the liver is just going to get worst and worst, until a point where he won't get better. The jaundice is a sign that his liver is already suffering, of course that was confirmed by the ultrasound.
You have to feed this kitty. Cats can absolutely not go without food as it will affect the liver - it will put them into a condition called fatty liver disease, or hepatic lipidosis.
Baby food is a good idea, but it has to be 100% meat, no garlic, no onions, nothing added - Beech-nut is a good brand.
Ask the vet what to feed him, if there is a food he will recommend for these cases, but ask fast - he can not wait much longer that he has already gone by without nutrition..... Ask if Hills A/D or if another recovery food would be good for him (it might be too strong, or not advisable with the blocked bile ducts)
Feeding him will mean more than a table spoon a day - it will mean syringe feeding a little bit at a time so he gets all the nutrition he needs for the day - at the very least, you want to feed him 50-75% of it. You can start with less for the first day or so as his system adjusts again....
Keep in mind, this is a vicious cycle - the loger he goes without eating, the less hunger he will feel, the less he will eat.... etc.... etc.... It is like the liver starts to get nutrition from within the body - you have to break the cycle.
I would be calling an emergency vet tonight to ask for instructions.... The e-vet you took him to might be able to help you since they already have his information on file....

Thank you so much for your reply. I have spoken to the vet and yes he is eating more. He is improving and I hope it continues! I will keep you updated. Keep sending those vibes because they are working!

Hi Carolina,

You are so sweet, thank you so much! I apologize for not writing sooner, I've been working like crazy for the past two days. I'm happy it's Friday!

Squeaky is continuing to eat more (still on the baby food) and he even started walking a little bit on his own! He didn't drink as much water today but I think it is because he is eating now. He has a great appetite. I will call the vet tomorrow to discuss a better diet for him, perhaps a vet rx diet that is gentle on his stomach. I'm not sure how long I should keep him on the baby food nutritional-wise.

He is a fighter! I also believe that all the prayers and good energy have a lot to do with his healing. I pray that he continues to improve.

I will keep you posted!
