I tried to vote but after I click "vote", the poll asks for my personal information. I declined. So I don't know if my vote was accepted or not.

Hmmm, have you "liked" our page first? I wonder if this might be it. My apologies for any inconveniences - this is my first time trying polling on Facebook. Setting up the poll was a pain, to be honest, so this may be a one-time thing, but I'm interested in seeing how this goes.

Its just an app, all apps ask permission first.
Anne, I voted and allowed the app, but it seems to be stalling after that, like it can't continue loading, though it is still trying.

I don't know what "app", but I only allow friends and family to see my details. Anyone or anything that asks for my details beyond my friends and family, I decline.
It's important for everyone to understand how "likes" work. I have heard an interview with computer experts, and the situtation is very complicated. The goal of Facebook is to give you information that will keep you on Facebook longer. (By the way, Google does this, too.) It uses your "Likes" to determine what to show you.
To give you an example, if a Democrat and a Republican do the same Facebook or Google search, they won't get the same results.

I thought only the page owner incorporating the app would need to allow it. Like Mr Blanche said, FB apps have to ask for your permission and you can either accept or refuse to user the app. It's no biggie for some people, while others are more careful. Since this particular poll app is the one Facebook officially recommends for using on pages, I think it's trustworthy, but I totally respect people who prefer not to use it.
FYI, we use FacebookConnect here on TCS, where you can connect your TCS account with your Facebook account. You can read more about here. I can tell you that as site owner, I can see if someone's account is connected, but I don't get any access to their FB page. The only thing the app does is grab the username and avatar from Facebook if you create your account through it - but you can still edit them yourself, of course. Later on, you can be logged in here if you're logged into FB and you can easily publish threads from here to your FB page.
I think that in the future we'll stick to local site polls - they seem to be working better and are easier to use. Thank you to all who voted - I'll go announce the winner now!
