In a previous post I explained that Ritz has been doing the litter box dance:
(1) around once a week, goes in the litter box, poops a little, goes back out, repeats around three times in 30 minutes; usually nothing much more comes out; (2) three to four days later, goes in the litter box, poops a little, goes back out, repeat multiple times (11 times in 90 minutes). In other words, the cycle repeats itself every ten days. She doesn't appear to be in pain, more frustrated. One time blood was on the stool, not in. Feces tested negative for parasites.
In between these times, she poops normally. Her stools are SOFT, not hard. A bit smelly.
The vet prescribed Flagyl for a possible GI upset, perhaps brought on by stress (this initially occurred shortly after I had several repairmen at my condo over a three to four day period). Flagyl didn't help.
Then the vet suggested increasing dry/wet to 50%/50%; I normally fed her 90% and 10% dry (Nutro Max Fish + Tomato sauce, the only kind of wet food Ritz will eat). That hasn't worked. It seems to have increased her appetite/hunger even though she is eating more calories, not less. It is shocking how much food she eats. I do not want her to gain any more weight (11.5 pounds a few weeks ago).
She plays normally, too--no change.
When I mentioned yesterday to the vet that a little urine comes out as well, the vet said, well, maybe it's a urinary problem. I am in the process of collecting a urine sample per the vet's request; he wants to rule out a UTI before doing more tests or trying more drugs.
I should add that she pees just fine at all times; the amount hasn't changed And that she almost always poops a normal (for her) amount the day before and after the cycle begins.
Ritz is female, around two years old; medium build; 11.5 pounds; a rescue from the streets cat in December 2009.
It is the PATTERN that I find fascinating, perplexing; and, of course, Ritz' discomfort. Any ideas what is going on with her? Thanks.

From what you're saying, it kind of sounds like a constipated cat. This website might give you some helpful info.

Questions:
-Why exactly are you feeding her tomato sauce?

-What are the color of your cats feces?

-Have you tried expressing her anal glands, or are they impacted?

-Do you go to the vet regularly and are up to date on vaccines?

-Have you tried to move the litter box, change litter, or change the box? (cats are picky)

My other suggestion is to see a different Vet. Just like doctors, a second opinion is always good. Plus they should do a cystocentesis to gather urine from your cat. It's easy. Unless her bladder wasn't full, then I could see them asking you to collect a sample.

I also want to add, that if your Vet is truly an experienced professional, he/she would let you know if this was a true concern.

Sounds like she's constipated. Is she dragging her hiney on the floor when she finishes? Have you tried feeding her plain canned pumpkin (not pumpkin pie filling) with wet food to soften her stools? How much water is she drinking? How old is she?

Thanks for your reply. I thought constipation was more related to the consistency of the stool; Ritz's stool is neither soft nor hard.

because it is in the Seafood and Tomato Bisque Max Cat food that she will only eat; although labeled "seafood", fish is the third ingredient.
-What are the color of your cats feces?
medium brown, neither soft nor hard; which is why I thought she WAS NOT constipated.

They were full and expressed at the vet for this problem, June 21-
Do you go to the vet regularly and are up to date on vaccines?
She goes at least once a year; last time prior to June 21, was for a dental cleaning around May 27. She had a yearly wellness check, including rabies, in early May. I'd take her more often (I'm paranoid about her getting sick) but she hates being picked up. She is a lap cat and sleeps with me all the time.-Have you tried to move the litter box, change litter, or change the box? (cats are picky) No (except the last four days when trying successfuly I might add to get a urine sample).

The vet is more puzzled than concerned. He recommened increasing Ritz dry food to 50/50 versus 90% wet, 10% dry--would that help with constipation.
I did notice that she rubbed her butt on the carpet yesterday. Feces tested negative for parasites twice this past month.
Ritz is strictly indoors, but I take care of a feral cat colony in front of my condo building. Any possibility she caught something from them?
Again I really thank you for taking the time to reply.

I didn't think she was constipated because her stools are normal, neither hard nor soft. Is this an incorrect assumption? Is this why the vet suggested increasing the amount of dry food/wet food to 50/50?
She did drag her hiney on the floor one or two days again. She is fed 90% wet, 10% dry. She hates pumpkin (tried that), sometimes will eat oat bran. She is almost two years old, female. I was able to obtain a urine sample from her;I'll get the results tomorrow. Thanks.

It does sound like she is constipated. Sometimes they can develop a big plug of hard poop in the bowel but they manage to poop "around it" regular looking stool. An X-ray would show this.

Urine sample showed a high level of white blood cells showing an infection and a rod-shaped bacteria (my terminology may be off). So Ritz will start a two week course of Baytril. The vet suspects a UTI. I suspect the UTI was stress-induced.
Anything I should know about Baytril? I'm getting the flavored tabs.
And, before this she was on a 10 day course of Flagyl (ended July 1) for the same symptons. Why wouldn't the Flagyl kill the bad bacteria?
Thanks.

Different bacteria? Flagyl (metronidazole) treats only anaerobic bacteria and certain parasites. Used for bacteria in the gut. It's like with people - the docs will prescribe a different antibacterial depending upon where the bacterial infection is.

Thanks. So, the high level of white blood cells indicates the system is fighing an infection, the infection being bacteria?
Should I be concerned/ask the vet what type of bacteria; all he said (voice mail) is "rod-shaped".

Hope so.
Would a routine urine analysis also show if Crystals were present?
I would hate to deprive Ritz of her one and only food: fish.

Yes, it would.

It would be a good idea to ask the vet specifically if there were any crystals, just to make sure there weren't any. Sounds like they would have mentioned it, but better to be sure.
