Feral Kitty health/behaviour problems

I rescued my kitty as a feral stray 9 years ago. It has taken a lot of work to get her to a "normalish" cat behaviour point - the little info I've managed to glean from rescue orgs is that it is unusual to have a feral cat become as domesticated as she has considering she is mostly indoors now - and she still exhibits quite a lot of feral behaviour. Recently she has become ill - lethargic, loss of appetite, diarrhea, and vomiting.
At the vet for 2 days and given an IV- fecal work - negative; blood work - negative ( except for fiv antibodies - now testing specifically for FIV - she was negative when I got her and she was vaccinated against it). Her gums/teeth need work but need to wait for health to clear up. She refused to eat at the vet and upon taking her home (I was given antibiotics) she ate no problem- she also ate when I came to visit her. Over the last day and 1/2 she refuses to eat - purrs and sniffs it but nothing. I've changed the food - wet/dry - different makers - straight tuna - all with no luck. She also has been gagging and this morning vomited bile. She also has been peeing outside her box- it is clean and there has been 0 change to her home environment.
Her behaviour - she was part of a extended family of feral cats, born in cold winter conditions, and the lowest on the totem pole, she is a very timid cat and submissive cat when around other cats and people - who she is generally afraid of. When she is nervous/afraid or whatever, she will usually eat without much fuss if I am nearby or petting her (i don't do this often). The 2 day trip to the vet was the first time she had been outside her home environment for any length of time, and the first time surrounded by strange noises, people, smells, and other pets.
I am really worried about her. I'm wondering if maybe she's not eating because she is so upset at being subjected to the vet? she ate for the first 3 days when she got back but this is becoming day 2 with no food. She drinks water, sniffs food and looks like she might eat, will lick a bit of tuna sauce but will not eat the tuna. She is very cuddly and purrs loudly - her eyes appear alert. Anybody have similar experiences or have any suggestions? I'm just really worried.

She sounds quite ill and I would if possible find a feline specialist in your area to see her. If you can't locate one, then find another vet, preferably one who specializes in small animals.

Probably, the cat reacts to stress (veterinary surgeon). After all she completely does not refuse meal, simply eats a little...
