I called and requested a form from the DMV there over two weeks ago, and have gotten nothing. It has to be sent by mail, as it's a triplicate form. I need it so my best friend, who has the car in question, can officially sell it and get something bigger.

If anyone can do this, I would be forever grateful. we've been trying to track down the title for this thing for ages... I bought it in NY, then moved to IL, then handed the car and payments off to her in Nevada... it's paid off... but I'm in Oregon... so we're getting headaches trying to get this resolved... as my name is still on the title.
