Question about registering my cat

Hi all!

I'm not a breeder, but I thought this might be the most appropriate board for my post.
I'm filling out the TICA paperwork to register my cat and am wondering where to put his name. There are boxes at the top to put the First Name Choice and the Second Name Choice, but the breeder filled those in with her cattery's name. Is this correct? I see more boxes for the Owner's Name...do I put my name there or is this for the cat? I don't see anywhere else to put it...

You put the nam next to the cattery name.
Put your first choice on the first line and the 2nd choice on the next line.
Do you see all the squares next to the Cattery name?
Put the name you want next to it.
You put your name on the owners name.
The cattery name is always before the pets name on the same line.
I just sent Wrinkles reg in last thurs and TICA emailed me her reg number today.

No problem.
I have filled out the blue slip before with my other sphynx.
What kind of cat did you get?
