Moving a former stray cat from the West Coast of California to Maryland

My cat was a stray when we found her out in the pouring rain if you can believe it was pouring rain in California. This was in February of 2008. It's now June 27, 2009 and it seems we will be going back to Maryland. We already found a place that is full of green grass and trees and lots of area to run around. This is a cat we found and took to the vet who told us at the time she was about 1 year old. We let her out all day and she comes back when we come home around the evening. So this is a cat who is wonderful with people, cat litter trained, usually goes outside though, and has never left this neighborhood. I've never owned a cat before this one but I'm getting married next month and we very much want to keep her after we move in January 2010. I'm wondering how do you move a cat-plane, car, what? She pooped in the car the last two times we took her to the vet as she was freaking out over the car ride even though it was 2 miles away. So I don't know how she'd make it 3000 miles away without pooping all over herself or having a heart attack along the way. She's very sweet and we want to keep her.
The next question is once we are in Maryland how do we retrain her that this is now home? Is it possible or do we have to keep her inside always from now on? That may be hard for her since she's very active and runs around all day outside.
We just want to know if it's even worth the try for her sake. Your thoughts?

Since she obviously dislikes car rides a lot, it may be better for her (she'll still get worked up about it) to fly commercial with a family member, and get it over in 5-6 hours vs. driving with a stressed out cat cross-country for several days.
