Need help on wording a gift certificate

The cat rescue group that I volunteer for is having a fundraiser at the end of the month.

So far I have knit three scarves, donating a piece of cat furniture, got some gift certs from my pt job.

I also want to donate an hour of garden consultation and 10 perennials from my gardens plant collections that I sell. These I would like to include a mix of shade and sun perennials.

I told the event planner that I would work up some sort of gift certificate
for this but am unsure of the verbage I should use.

I was thinking of putting an expiration date of mid June on it as the plants should really get in the ground before the summer heat comes.
Also I will include a partial list of plants for thought starters.
And a bit about my business (not sure on that).
A approximate value of this certificate?? Value will be at least $100 or more depending on the plants.

I need to drop off by next Thursday.

I don't know wording, but some things you might want to include in the wording are:

1. An expiry date for the service to be used by;

2. "The bearer of this certificate". That keeps people from saying "I won" and taking advantage of the service even when they didn't win. If they have to present the certificate to you, then you know it's the actual winner, and that it's not going to be reused.

3. The amount of free time the person gets with you.
