Adoption form?

Does anyone have any adoption forms and agreements that they have used that they might be able to post or email me so that I can use them for when my kittens get adopted?

I am wanting to have a couple "back-ups" just in case my potential adoptor falls through so next week I will be having some people fill out forms, but I have no clue what to say on the forms, lol.

Two websites that might be able to give you excellent information: http://www.bestfriends.org and http://www.deltarescue.org (I hope the second one is right; if not, GOOGLE "D.E.L.T.A. Rescue" and it is in Acton, California. They put out an excelent book, "IS THIS THE PLACE?" all about how to successfully screen potential adoptees and rescue animals. 1. I agree to provide the Adopted Animals with necessary inoculations at the intervals advised by my veterinarian.

2. If for any reason I cannot keep the Adopted Animals, I agree to notify the Guardian of the availability of the pet and to return the Adopted Animal upon request.

3. I agree not to abuse or neglect the Adopted Animals and I authorize the Guardian, at her sole discretion, to determine whether or not the pet has been abused or neglected.

4. I agree to not declaw the Adopted Animals.

5. I understand that any failure to perform the foregoing agreement will constitute a breach of contract. In the event of any such breach of contract, I authorize the Guardian to reclaim both possession and ownership of the Adopted Animals.

6. I understand that the pet covered by this adoption contract is, as far as can be determined by the Guardian, in good health and that the Guardian is not responsible for any medical fees incurred after the adoption date. However, if a health problem develops during the first 10 days, I should notify the Guardian so we may discuss the matter.

7. I agree to give the Guardian occasional visitation rights to ensure that the terms of this adoption agreement are being observed.
