Toms have fat heads. Once they reach 8-12 months, they start to develop the jowls ect. If you want a cat to look that way, you shouldn't neuter them any younger. I have two toms and they don't spray. They're very affectionate as well, contrary to popular belief.

Yes, it is better to take the weight off slowly. About the kitty being fat and dying happy, that's sadly not gonna happen.....He'll get arthritis and suffer terribly especially as he ages. He may have to put to sleep early as well. Not happy....

Urinary tract problems are VERY common once a male cat is neutered. I've heard many cases like yours. Unfortunately, you may just have to keep him on special care the rest of his life, off and on or full time. I wish you luck, and hope Teddy gets better.
